You will receive a confirmation email once your submission is successful. After July 8, 2019, the areas of bus service will be determined and communicated to you by Mallard Creek Stem Academy. Please enter your information and your student’s information as directed on the form. You will receive a confirmation email that your submission was successful. If you need to update or revise the information provided, you will be able to do so via the link provided in the confirmation email you will receive. Providing the information does not guarantee bus service, nor does it establish a bus stop near the address entered. The purpose of this Portal is to gather information as to the need for bus services and to determine the possible areas of bus service. Mallard Creek Stem Academy has established a cut-off date for receipt of this information. Once that date has passed, the information provided to the cut-off date will be given to MCSA for the establishment of service areas. If you have any questions, regarding this form, it’s use, or the establishment of service areas, please contact your school. Olympic will not sell, convey or transfer any email addresses or personal information obtained on this form to any third party.
